NOW COMES the Borough of M chanicsburg, Cumberland County,
Pennsylvania, by its staff attorney Davi J. Spotts, Esquire, and files the within claim for
charges assessed with respect to sew r and/or refuse rates against above named
owner or owners and against the herei after described real estate as follows:
STATE ENT OF CLAIM
1. This claim is filed by the ,oroU9h of Mechanicsburg. a borough organized
and existing in accordance with the la~s of the Commonwealth of Pennsylvania.
2. The owner or reputed ower of the property. or party or parties
responsible or liable. for which this clai is filed is the Defendant(s).
3. The authority under whic the sewer and/or refuse rates were charged
and assessed is the Act of July 18, 19 5, P. L. 1286. Section 1, 53 P .s. 9 2231, as
amended.

4. The authority under which this claim is filed is the Act of May 16, 1923,
P.L. 207, Section 9, et seq., 53 P.S. 137143, as amended.
5. A description of the prop+y against which this claim is filed is:

6. This claim has not been t e subject of any previous claim and judgment in
the Court of Common Pleas of Cumbe land County for sewer and/or refuse charges.
7. The total amount of the c arges for which this claim is filed is four
hundred and forty-five and 50/100 doll rs ($445.50), plus costs and reasonable
attorney's fees.
8. The time period for which these charges are filed is January 1, 2005 to
March 31. 2006 on Borough Sewer Ac OU[lt(s) 00-0100100.
WHEREFORE. Claimant reque ts that a Municipal Lien be entered in its favor
and against the Defendant in the amo nt of four hundred and forty-five and 50/100
dollars ($445.50), plus costs and reas nable attorney's fees.
