PETITION TO CHANGE NAME
AND NOW comes Petitioner Denise M. Starner, by and through her attorney
Pamela L. Purdy, Esquire and files this Petition to Change Name on behalf of her
minor son, Andru Charles Appleman, and in support thereof, avers as follows;
1. Petitioner is Denise M. Starner ("Mother"), an adult individual currently
residing at 44 Nottingham Drive, Mechanicsburg, Cumberland County,
Pennsylvania.

2. Mother has resided at the following addresses during the five years
prior to filing this petition:

3. Mother is the biological mother of Andru Charles Appleman, born
November 29, 2004.
4. Mother desires to change Andru's name from Andru Charles
Appleman to Andru Charles Starner.
5. Mother believes that changing Andru's name would be in his best
interest for the following reasons:
a. The surname "Appleman" is the surname of David C.
Appleman who was believed to be Andru's biological father at the time of his
birth.
b. Mother and Mr. Appleman since have learned through two
paternity tests that Mr. Appleman is not Andru's biological father. Mr.
Appleman has signed an Acknowledgment of Non-Paternity, which is
attached hereto as Exhibit "A."
c. Andru no longer has a relationship with Mr. Appleman.
d. Andru lives with and is being raised solely by Mother, whose
surname is Starner.

e. Andru will suffer emotionally, socially and morally from the
stigma of having the surname of a man with whom he has no relationship and
will have no relationship in the future.
f. Andru will benefit emotionally, socially and morally from having
the same surname as Mother.
6. No credit has been extended to Andru since his birth under the name
of Andru Charles Appleman, and that this change of name is not requested for the
purpose of concealment of identity or for any fraudulent purpose.
WHEREFORE, Petitioner Denise M. Starner respectfully requests that this
Honorable Court enter a decree changing Andru Charles Appleman's name to Andru
Charles Starner.

I, DAVID C. APPLEMAN, do hereby acknowledge that I am NOT the father of
the child, ANDRU APPLEMAN, born on November 29, 2004. This child was born to
Denise M. Stamer of Mechanicsburg, Pennsylvania.
I verify that the statements made in this documetns are true and correct to the best
of my knowledge, information and belief. I understand that false staemetns are made
subject to the penalties of 18 Pa.C.S. S 4904, relating to unsworn falsification to
authorities.

ORDER OF COURT
AND NOW, this 30th day of March, 2006, upon consideration of the attached
Petition for Name Change, a hearing is scheduled for Friday, June 2, 2006 at 9:30 a.m.,
in Courtroom No.5, of the Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to
the hearing; in addition, Petitioner shall comply with all other requirements of the
C.C.R. P. 5, and the Act of December 16,1982, P. L. 1309, ~6 (see 54 Pa. C.S. ~701
[Historical and Statutory Notes] [Main Volume]), and 54 Pa. C.S. ~~701 et seq.

IN RE: NAME CHANGE
ORDER OF COURT
AND NOW, this 2nd day of June, 2006, following the
hearing, IT IS ORDERED that the name of the child Andru
Charles Appleman is changed to Andru Charles Starner.
IT IS FURTHER ORDERED AND DIRECTED that the petitioner
is authorized to remove David Appleman's name from the birth
certificate of the child, the Court having been shown that
David Appleman is not the biological father of Andru Charles
Starner.
