Petition in the above-referenced matter to create and fund a Special Needs Disability Trust for

the benefit of BARBARA A. FARROW, it is hereby'ORDERED, ADJUDGED and DECREED

that the Special Needs Disability Trust for the benefit of BARBARA A. FARROW, is
established and approved in the form as presented to the Court, and Joyce F. Gifford, in her
capacity as Trustee, is hereby authorized to execute the trust agreement, thereby agreeing to be
bound to the terms and conditions thereof. It is further ordered that Joyce F. Gifford shall have
the authority, in her capacity as Trustee, to fund said Special Needs Disability Trust for the
benefit of BARBARA A. FARROW with the remaining principal and income from BARBARA
A. FARROW's inheritance from her deceased mother's estate and any additional funds received
by BARBARA A. FARROW from any other source(s).
