PETITION FOR CHANGE OF NAME Petitioner, George Eric WUkins, by and through his attorney, Nathan e Wolf, Esquire, hereby presents this petition requesting that this Court order a change of petitioner's name, pursuant to 54 Pa. es. ~ 701 et seq., and in support thereof makes the following averments: 1. Petitioner was born on March 28,1954, and is over the age of 21 years. 2, During the five years preceding the filing of this petition down to and including the date thereof, petitioner has resided continuously at the following locations: (a) 33 South West Street, Carlisle, Pennsylvania 17013 - 2003 -Present; (b) 216 South 4m Street, Mt. Wolf, Pennsylvania 17347 - 2001-2003; and, (c) 215 Hempt Road, Mechanicsburg, Cumberland County, Pennsylvania, 17055- 1997-2001. 3. There are no judgments or decrees of record or any other matters of like character against the petitioner. Certification of an official search is attached hereto as Exhibit "A", 4. Petitioner requests that this name be changed from George Eric WtIkins to Eric George WtIkins. 5. The reasons forthe requested change of name are the following: (a) Petitioner has been known as Eric WIlkins informally for his entire life, and was called by that name by his family, friends, and co-workers; (b) Petitioner has used G. Eric WIlkins for formal submissions at all possible events, which is now being frustrated by recent information technology changes, including a requirement to have his full name on his driver's license; and, (c) Petitioner wishes to change his last name prior to his remarriage, scheduled for July 1, 2006. 6, If approved, Petitioner's request will merely change the order of his given first name and middle name, which, as stated above, has been the name Petitioner has used all of his life. WHEREFORE, petitioner, George Eric WIlkins, respectfully prays that this Honorable Court enter an order fixing a time for a hearing on the instant petition before this Court, no less than one month, nor more than three months from the filing of the instant petition, and that after hearing, that this Honorable Court issue an Order authorizing the change of petitioner's name to Eric George WIlkins, along with any additional relief that the Court may deem appropriate.
