Jefferson, by himself and respectfully files the following Motor Vehicle Registration Suspension Appeal as follows: 1. Petitioner, James R. Jefferson is an adult licensed driver with a residenc address of 1705 Brandt Ave, new Cumberland, Cumberland County, Pennsylvania 17070. 2. The Department of Motor Vehicles has an address of P.O. Box 68674, Harrisburg, PA, 17106-8674. Petitioner received notice of a proposed 90 day suspension for failure of insurance. 3. I wish to appeal this based on the following. a. When the original insurance was taken out it was to be paid by my sisters credit card monthly as I had just moved here after retiring from the U.S. Navy and she was helping me out. Somehow this was not recorded with the Insurance Company. b. I do not recall receiving notice of cancellation. c. Said vehicle is insured and has been since shorthly after this notice was received and is on an automatic payment from my checking account. d. We are a one car family with medical and mental health issues with our children and have no other means of transportation for these or work. Either no response was received or the infonnation you provided was not acceptable. As a result, the registration for the vehicle listed above will be suspended for three months effective 04/11/06 at f2:01 A.:\1. as authorized by Section 1786(d) of the Vehicle Code. Credit towlIrd serving this suspension will not begin until the Depllrtment receives your registration products. Additionally, you are required to pay a restoration fee in the amount of $50 to the Depal1ment in accordance with Section 1960 of the Vehicle Code.
