PETITION FOR NAME CHANGE.
AND NOW, comes Meagan A. Gunderson, by and through her Attorney, Jane Adams,
Esquire, and respectfully requests this Honorable Court to change her name to Meagan A. Rouse
for the following reasons:
1. Petitioner, Meagan A. Gunderson, (hereinafter referred to as "Petitioner") currently
lives at 4126 Kittatinny Drive, Mechanicsburg, Pennsylvania, 17055.
2. Petitioner's date of birth is March 4, 1987.
3. Petitioner lives with her natural mother, Nancy Rouse, and her step-father, William L.
Putt.
4. Nancy Rouse and William L. Putt were married on December 31, 1999; Nancy Rouse
chose to retain her maiden name after the marriage.
5. Petitioner's natural father, R. Scott Gunderson, is currently believed to reside at 468
Island Road, Bossier City, Louisiana, 71111, with his wife, Belinda Johnson.
6. Petitioner has very limited contact with her father, R. Scott Gunderson.
7. Petitioner desires to use her mother's maiden name since she resides with her mother.
8. Petitioner believes that using her other s malden name would avmd confusion
regarding school and medical records.
9. Petitioner has no outstanding credit debt that could be avoided through a name change.
10. Petitioner has no outstanding federal or state tax debt that could be avoided through a
name change.
WHEREFORE, Petitioner requests that this Honorable Court Order that her name is
changed to Meagan A. Rouse.
