the following inventory constitutes a true and complete statement of all assets of the
incapacitated person that have come into the possession of the guardian, with the estimated value
of each asset, together with a statement of any additional assets of the incapacitated person that
the guardian expects to acquire in the future. The undersigned guardian understands that false
statements made in this inventory are subject to the penalties of 18 Pa.C.S. ~ 4904 relating to
unsworn falsification to authorities.
