PETITION FOR CHANGE OF NAME The Petitioner, Bernie V Ward, through the undersigned Counsel, requests that this Honorable Court order a change of name pursuant to 54 Pa. C. S. 9 701, et. seq., and in support hereof the following: 1. Bernie V Ward is a resident of Carlisle, Cumberland County, Pennsylvania, and has resided at the following locations during the five years next preceding the filing of this Petition: a. August 2004 to Present - 141 S. Pitt Street, Carlisle, Cumberland County, Pennsylvania 17013; b. October 2002 -- August 2004 at 129 S. Second Street, Philipsburg Borough, Centre County, Pennsylvania; c. August 2001 --October 2002 at 313 12th Street, Huntingdon, Huntingdon County, Pennsylvania and d. February 1999-August 2001 at 1271 W. Gatesburg Road, Warriors Mark, Ferguson Township, Centre County, Pennsylvania. 2. Petitioner was born on December 30, 1951 in New Brunswick, New Jersey and Petitioner's natural parents are the late Bernard Park Ward and Joan Armstrong Ward. 3. Petitioner requests a change in name from Bernie V Ward to Abigail Elizabeth Reynolds. 4. Petitioner desires the requested change of name for the following reason: a. Petitioner likes the name and prefers it to be the legal one. 5. There are no outstanding judgments or pending legal actions of record against Petitioner. 6. Petitioner has never been convicted of a felony nor has Petitioner been convicted of any crime itemized in 54 Pa. C. S. ~ 702 (a) (2).
