Rovegno Secrefary Terry L. Barley Director ANNUAL GUARDIANSHIP REPORT FOR MARY DUGAN Report from Guardian of Person Cumberland County Office of Aging Priscilla M. Whitman, Aging Care Manager III On May 19, 2005, Permanent Guardianship of Person for Mary C. Dugan was granted by Judge Kevin Hess to Cumberland County Office of Aging. Prior to this appointment Mrs. Dugan resided in her own home at 308 Mt. Allen Rd. Mechanicsburg, Pennsylvania. She was admitted to The Bridges at Bent Creek in Mechanicsburg on April 26, 2005 after the outcome of an Emergency Guardianship hearing. The Bridges at Bent Creek is an assistive living facility. At the time of the Permanent Guardianship hearing Brian Brooks of the P A Guardianship Association was appointed Permanent Guardian of Estate. Judge Hess on November 28, 2005 approved the withdrawal of P A Guardianship Association and appointed Keystone Guardianship Services as Permanent Guardian of the Estate of Mary Dugan. Mrs. Dugan upon admission to The Bridges had been diagnosed with depression, hypertension, arthritis, GERD, hyperlipdemia and Senile dementia Alzheimer's type. Her medications are: Darvocet, Lexapro, Prilosec, Toprol XL, Norvasc and HCTZ. Mrs. Dugan's cognitive status over the past year has deteriorated especially her short term memory. Due to this she was moved into the dementia unit which better meets her needs. She requires 24 hour supervision and some hands on assistance with her activities of daily living. Staff provides the necessary verbal cues and redirection when required. Mary benefits from the dementia unit activities that are more appropriate to her dementia diagnosis. Mrs. Dugan has been emotionally stable and has not displayed any behavioral problems. Mrs. Dugan has done very well over the past year and adjusted easily to her new living arrangement. She attends all the different social activities on a4~ily basis. MarYl'egjoys her surroundings, the staff and the other residents. She is always very p;~~~a!lt,.h~.~~@1ile on her face and enjoys joking around with others. She has made friends and fQels'~y~iy..:secure in her living environment. Mary does not like being away for any length of time and is always relieved when she returns. The staff enjoys having her and she is c~et}l~i~ ~thi\l1~ftJOOZ .AOO ~- bJfb L ~ requests. Mrs. Dugan has been in good health overall and appreciates the meals. She has put on weight due to a good appetite. Client is visited every 30-60 days by care management staff. The Office of Aging provides scheduling and transportation to all her medical appointments. The staff at The Bridges provides updates and notification of any changes that Mrs. Dugan might experience. Connie Stoneroad of Keystone Guardianship Services also visits every 30-6- days. A few former neighbors visit regularly and family visit occasionally which suits Mary. It is the request of the Cumberland County Office of Aging that the Agency retains Guardianship of Person for Mary Dugan. The Bridges at Bent Creek has demonstrated an adequate quality of care for client by managing her daily needs and continuously monitoring her medical conditions. Mrs. Dugan's appearance is always good and she appears to be very content in her present living facility. Therefore, we believe that Mrs. Dugan should continue to reside at The Bridges of Bent Creek.
