VERIFICATION OF SERVICE
I, Margaret A. Fritz, hereby verify and state that:
1. I am a long-time friend of Shirley A. Gingerich
and am therefore authorized to make this verification.
2. On 1"lA..'f ~ , 2006 at approximately S: 00
~.m., I personally served a copy of President Judge Bayley's May
JL, 2006 final decree and accompanying statement of rights on the
incapacitated person, Shirley A. Gingerich. I reviewed the final
order and statement of rights with Ms. Gingerich, and then, to
the maximum extent possible, explained the documents to her, in
language and terms she was most likely to understand.
3. The facts contained in this verification are true
and correct to the best of my knowledge, information, and belief.
4. I understand that false statements herein are made
subject to the penalties of 18 Pa. C.S. ~4904, relating to
unsworn falsification to authorities.
