MECHANICS' LIEN CLAIM
1. Claimant is Mellon Certified Restoration hereinafter referred to as "Claimant".
Defendant/Owner(s) are Richard Ben & Sharon Fahrer Hauschildt, who reside at 5248
Strathmore Drive, Mechanicsburg, PA. 17055, hereinafter referred to as
"Defendant/Owner".
The property of the Defendant/Owner that is subject to the mechanics' lien claim by the
Claimant, is the property and improvements thereon located at and commonly known as
5248 Strathmore Drive, Mechanicsburg, PA. 17055.
The emergency services completed by Claimant, was done pursuant to a written contract
entered into on December 25, 2000 between Claimant and Defendant/Owner which is
attached as Exhibit "A" for our agreed sum of $ 19,848.66, of which a copy of the
customer invoice is attached as Exhibit "B", by the Claimant for the Defendant/Owner.
The repair and restoration was completed by the Claimant and was done pursuant to a
written contract entered into on December 26, 2000 between Claimant and
Defendant/Owner which is attached as Exhibit "C" for our agreed sum of $ 268,387.51
by the Defendant/Owner.
On December 3, 2001, the Claimant had performed the repairs due to the fire loss, as a
building contractor in connection with the restoration of the home at the aforementioned
property owned by the Defendant/Owner.
The labor and materials subject to a mechanics' lien claim pursuant to the Mechanics'
Liens Law, act of August 24, 1963, PL 1175, No. 497, Section 1503 (6); 49 P.S. Section
1503 (6) which were provided by Claimant are those described in the itemized scope and
pricing set forth from Claimant to Defendant/Owner, which is attached hereto as Exhibit
8. Pursuant to the aforesaid contract, the Claimant had completed the restoration on
December 03, 2001, on the aforementioned property.
As the consequence of the labor and materials furnished by the Claimant the total for the
restoration was $ 288,236.17, The Claimant was paid $ 107,909.35, which is hereto
attached as Exhibits "E" and "F", and is owed the sum of $180,326.82, of which copies
of the customer invoices are attached, hereto as Exhibits "G", "H" and"I".
