Commonwealth of Pennsylvania by its solicitor, Broujos & Gilroy, P.C., hereby fIles its claim
for water and sewer services against the real estate hereinafter described and sets forth its
claim as follows:
STATEMENT OF CLAIM
1
The name of the municipality by which this claim is filed is North Middleton Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name or names of owner or owners or reputed owner or owners, of the property against
which this claim is filed is Charles L. Carothers and Emma L. Carothers for 100
Meadowbrook Road, Carlisle, Cumberland County, Pennsylvania.

A description of the property against which it is fIled is attached hereto and marked Exhibit
"A", with an address of 100 Meadowbrook Road, Carlisle, Pennsylvania.

The authorities under and by virtue of which this lien is fIled is the Municipal Claims and
Liens Act of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid water and sewer hook-up fees pursuant to an agreement
between the parties against the above described property in the amount of $7,635.00.

that certain tract of land situate in North Middleton
Cumberland County, Pennsylvania, bounded and described as
BEGINNING at a point in the centerline of the Public Road
known as Meadowbrook Road which leads from the Carlisle-Newville
Road to Meadowbrook Park, said point at the Place of Beginning
being the Northeast corner of land now or formerly of Paul E. Unger
and wife; thence from said point at the Place of Beginning along
line of said land now or formerly of Eaul E. Unger, North 88
degrees West a distance of 140 feet to a point in line of land now
or formerly of George H. Myers, Jr.; thence along line of said land
now or formerly of .George H. Myers, Jr., North 02 degrees East, a
distance of 75 feet to a point in line of land now or formerly of
Lois N. Downes; thence along line of said land now or formerly of
Lois N. Downes, South 88 degrees East a distance of 140 feet to a
point in the centerline of said Meadowbrook Road; thence along the
centerline of said Meadowbrook ROGd, South 02 degrees West a
distance of 75 feet to a point at the Place of BEGINNING.
THE ABOVE described tract of land contains 75 feet in front
along the centerline of Meadowbrook Road and extends westwardly
therefrom at an even width a distance of 140 feet to land now or
formerly of George H. Myers, Jr., and has thereon erected a one-
story dwelling house and other improvements known as and numbered
100 Meadowbrook Road, Carlisle, Pennsylvania 17013.
THE ABOVE described tract of land is conveyed under and
subject to the same building and use restrictions set forth in the
deed from Frank S. Cohick and Lois J. Cohick, his wife, to Clair E.
Myers and Betty H. Myers, his wife, by deed dated November 29,
1952, and recorded in the hereinafter named Recorder's Office in
Deed Book nDn, Volume 15, Page 302.

THE WITHIN conveyed tract of land is all of the property which
Lois J. Downes and husband, by deed dated April 7, 1967, and
recorded April 7, 1967, in the Office of the Recorder of Deeds in
and for Cumberland County at Carlisle, Pennsylvania, in Deed Book
"H", Volume 22, Page 335, granted and conveyed to Emmett E. Myers
and Helen Irene Myers, husband and wife, Grantors herein.
