PETITION FOR CHANGE OF NAME
Petitioner, DEANNA BOLES, on behalf of her minor son, Jaren Anthony Patton,
requests that this Court order a change of name for her son, and in support thereof, represents as
follows:
1. Petitioner, Deanna Boles, is an adult individual residing at 158 S, Enola Drive,
Enola, PA, Cumberland County, Pennsylvania 17025,
2, Petitioner is the biological parent of one child, Jaren Anthony Patton, a minor
born on March 15, 2004, Jaren is 2 years of age,
3, Petitioner has resided at 158 S. Enola Drive, Enola P A 17025,
4, The biological fatller of Jaren, Standrick 1. Patton, who resides at 171 Lepore
Drive, Lancaster, P A 17602.
5, The child was born out of wedlock,
6. The child has always lived exclusively with the Petitioner.
7, The child has resided continuously with Petitioner and his brother Nathaniel V,
Boles his entire life,
8, Petitioner, on Jaren's behalf, requests the change of surname from Patton, natural
father's surname, to Boles, his mother's name, changing his current surname from Patton to
Boles,
9, Petitioner believes and, therefore avers, that the change of her son's surname from
Patton to Boles is entirely consistent with Jaren's best interests so that Jaren can share the same
last name as his mother and brother,
10, Jaren has no connection with his biological father, and has had limited contact
with him since approximately birth.
11. Petitioner has contacted Mr. Patton, who consents to this requested change of
name, as indicated by his consent thereto, which is attached to this Petition as Exhibit "A,"
12, Petitioner firmly believes that the requested name change is in the best interests of
the child and truly wishes to have the change decreed by this Court,
WHEREFORE, Petitioner respectfully requests that tllis Honorable Comi enter a decree
changing the name of her son from Jaren Anthony Patton to Jaren Anthony Patton Boles,

CONSENT TO CHANGE OF NAME
I, Standrick L. Patton, am an adult individual who resides at 171 Leoore Drive,
Lancaster, P A 17602, I am the biological father of Jaren A, Patton; born March 15,
2004, in Harrisburg, Pennsylvania,
The Mother of the Child is Deanna Boles, who resides at 158 S, Enola Drive,
Enola, Pennsylvania, and is the custodial parent and guardian of the Child,
I hereby voluntarily and unconditionally consent to the change of name of the
above-named child,
I understand that Deanna Boles will petition the Cumberland County Court of
Common Pleas for the change of name of the child, and that I will be notified in writing
of the date of the hearing and that should I have any objection to this change of name, I
may state so at that hearing.
I understand that I may not revoke this consent after a court has entered a decree
granting the change of name,
