of the Citation and Petition was made on Constance Meredith by having a copy of it read to her,
as well as hand delivering a copy to her, on May 17, 2006, at 1 :50 p.m. at the Danville State
Hospital, located at 200 State Hospital Drive, Danville, Pennsylvania 17821. The Petition and
Citation were read and explained to the alleged incapacitated person, to the maximum extent
possible, in language and terms she was likely to understand.

determine this matter is scheduled on June 12, 2006 at 2:30 p.m. in Courtroom No.3 in the
Cumberland County Court, Orphan's Court Division, One Courthouse Square, Carlisle,
Pennsylvania.
