Johnson, Esquire, requests that this court order a change of name for her minor child, Brycen Nicholas Weibley pursuant to 54 Pa. CONS. STAT. Section 70 I et. seq, and in support thereof respectfully represents as follows: 1. Petitioner is an adult individual and the natural mother of Brycen Nicholas Weibley born May 30, 2002. 2. At the time of the child's birth the name Brandon Weibley was placed on the birth certificate as the father and the child was given the last name ofWeibley. 3. As a result of genetic testing the Cumberland County Domestic Relations Section has conclusively presumed that Brandon Weibley is not the biological father ofBrycen Nicholas Weibley. A copy of the genetic test report along with a copy of a letter from Domestic Relatoins is attached hereto and made a part hereof. 4. Brandon Weibley does not desire to have the child have his last name and consents to the granting of this Petition. A copy of his consent is attached hereto and made a part hereof. '. ~ 5. The child has lived at 10 Jane Lane, Carlisle, Cumberland County, Pennsylvania from May 30, 2002 until November 2004; 5 Winchester Gardners, Carlisle, Cumberland County, Pennsylvania from November 2004 until November 2005; and 10 Jane Lane, Carlisle, Cumberland County, Pennsylvania from November 2005 the present. 6. Petitioner requests that the child's name be changed from Brycen Nicholas Weibley to Brycen Nicholas Stimeling.
