COMPLAINT
1, Plaintiff, Chase Manhattan Bank, is a corporation located i Wilmington, Delaware,
2, Defendant is an adult individual with a last known addre s of 394 Brick Church Rd,
Enola,Pa 17025.
3. On or before May 4, 2003, Plaintiff and Defendant ntered into a Cardmember
Agreement for a credit card bearing the account number 426690 I 0219961 8. A true and correct copy
of the Cardmember Agreement is attached hereto, marked as Exhibit "I" made a part hereof.
4. The Cardholder Agreement contains a provision to settl by arbitration any claim,
dispute or controversy arising from or relating in any way to the Cardholde Agreement.
5, Defendant made use of said credit card and has currently balance due and owing to
Plaintiff, as of March 3, 2006, in the amount of $5,909.65.

6, Defendant is in default of the terms of the cardholder greement having not made
monthly payments to Plaintiff thereby rendering the entire balance immedi tely due and payable,

7, Plaintiff avers that the Cardholder Agreement betweeb the parties provides that
Plaintiff is entitled to the addition of finance charges at the rate of 6,0% per annum on the unpaid
balance.

8, Although repeatedly requested to do so by Plaintiff, D fendant has willfully failed
andlor refused to pay the principal balance, finance charges or any part the eofto Plaintiff,
WHEREFORE, Plaintiff demands Judgment in its favor and ag inst Defendant, Stepanie A
Hartman individually, in the amount of $5,909.65 with continuing finance harges thereon at the rate of
6.0% per annum from March 3, 2006 plus costs; or in the alternative, an rder directing the parties to
arbitrate this dispute pursuant to the terms contained in the Cardholder Agr ement.
