PETITION OF ALYSSA MORGAN AUGUSTINE-LO~NCE FOR THE CHANGE OF
HER NAME TO ALYSSA MORGAN LORANCE;
1. Petitioner Alyssa Morgan Augustine-Lorance is a resident of Mechanicsburg,
Cumberland County, Pennsylvania. From May 2001 to April 2002, the petitioner
resided at 810 Samoset Drive, Harrisburg, Pennsylvania. From April 2002 to the
present, the petitioner has resided at 178 Stonehedge Lane, Mechanicsburg,
Pennsylvania 17055.
2. The petitioner was born on August 10, 1992 in Cumberland County,
Pennsylvania. Her parents are Bradley Clay Lorance (father) and Mary Kathleen
Hughes (mother).
3. The petitioner and her parents reside at 178 Stonehedge Lane, Mechanicsburg,
Cumberland County, Pennsylvania.
4. Petitioner would like to change her name from Alyssa Morgan Augustine-
Lorance to Alyssa Morgan Lorance, because she wants to pay proper tribute to her
adopted father and no longer feels that it is necessary to perpetuate the name
Augustine.
5. Your Petitioner is not married and has not mothered any children in her lifetime.
6. There are no judgments or decrees of like character of record against the
Petitioner.
7. A certified copy of the petitioner's birth certificate is attached hereto marked

Exhibit B and incorporated herein by reference. A fingerprint card has also been
provided for the petitioner.
Wherefore, Petitioner requests thi~ Honorable Court to set a hearing to consider
the requested change of name as contained herein, to ultimately enter an Order
changing Petitioner's name from Alyssa Morgan Augustine-Lorance to Alyssa Morgan
Lorance, and to provide any other relief this Court deems appropriate.
