PETITION FOR CHANGE OF NAME
AND NOW, comes Allison Marie Wetzel, by and through her attorneys, The Shagin Law
Group LLC., and petitions the Court as follows:
1. Your Petitioner is Allison Marie Wetzel, an adult individual whose address is 4301
Woodcrest Lane, Camp Hill, Cumberland County, Pennsylvania, 17011; a copy of her
Certification of Birth is attached hereto and made a part hereof marked as Exhibit "A."
2. Your Petitioner desires to change her name to Allison Marie Shertzer, to change her last name
to that of the maiden name of her mother.
3. The Petitioner does not have any record judgments against her in Cumberland County, or in
any Court in any jurisdiction of the United States of America.
4, Your Petitioner files this Petition in good faith, and not for the purpose of avoiding any
creditors, and has no judgments filed against her in Cumberland County nor any other
county.

5. Your Petitioner does not have a criminal record against her in Pennsylvania, nor in any
Court of any jurisdiction of the United States of America; a copy of the Fingerprint Card
is attached hereto and made a part hereof marked as Exhibit "B,"
WHEREFORE, your Petitioner requests Your Honorable Court to grant her Petition so
that she may now be known as Allison Marie Shertzer.

In accordance with the Name Change Act of 1998, the fingerprint cards of the above named individual, who
has applied for a name change, has heen searched in the files of the Pennsylvania State Police, Criminal Records and
Identification Division Central Repository, and:

) This person is subject to 18 PA C.S, Chapter 91, and the name change has heen noted on the person's
criminal history record information.
( ) A criminal record was revealed containing felony convictions. IN ACCORDANCE WITH THE NAME
CHANGE ACT, THE COURT SHALL NOTIFY THE PENNSYLVANIA STATE POLlCE WHEN A NAME
CHANGE FOR A PERSON CONVICTED OF A FELONY HAS BEEN ORDERED, The Pennsylvania State
Police, upon receipt of this notice, shall include the change of name information in the Central Repository as
provided for in 18 PA C.S., Chapter 91 (relating to criminal history record information).
( ) This person has been convicted of felony vioIation(s) for which the coun may not order a change of name,
( ) A criminal record was revealed that might prohibit a name change. Because the subsection(s) of the
affected conviction(s) has not been provided to us, we are unable to determine whether a name change can

You are also advised that the above response is based on comparison of fingerprints and associated request~r-
furnished infonnation against a name index and fingerprints contained in the files of the Pennsylvania State Police

for Change of Name, a hearing is scheduled for Monday, October 2, 2006, at 1 :30 p.m.,
in Courtroom No.1, Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, ~6 (see 54 Pa. C.S. ~701 [Historical and
