PETITION TO ADJUDICATE INCAPACITY AND
APPOINT A GUARDIAN OF THE PERSON/EST ATE
1. Petitioners are Roger Keith Potteiger, an adult individual residing at 669 Lindsey
Road, Carlisle, Cumberland County, Pennsylvania 17013; and, Randy Jo Potteiger, an adult
individual residing at 137 Yellow Breeches Drive, Camp Hill, Cumberland County, Pennsylvania
17011.
2. The alleged incapacitated person, Doris Ann Potteiger, has a
current residence at 20 North 12th Street, Apt. 218, Lemoyne, Cumberland County, Pennsylvania
17043. Ms. Potteiger is 80 years of age with a birth date of May 3, 1926.
3. Ms. Potteiger's current heirs at law are as follows:

4. Ms. Potteiger's property consists of the following monthly receipts:
Social Security benefits $629.00
Federal Government Pension $1,065.00
State Government Pension $392.99
5. No Court has ever assumed jurisdiction to determine the incapacity of Doris Ann
Potteiger.
6. Petitioners request the guardianship due to Ms. Potteiger being incapable of
making decisions in reference to her estate or person. Ms. Potteiger is unable to perform her
activities of daily living without assistance and supervision.
7. No less restrictive alternatives are available to adequately provide for the physical
and financial care of Ms. Potteiger.
8. The Petitioners request that a plenary guardian be appointed of the person and
estate to manage Ms. Potteiger's financial, medical and health care affairs, and the power to
make decisions regarding medical treatment and life support. Petitioners suggest that they be
appointed Guardians of the Person and Guardians of the Estate. Petitioners are the sons of Ms.
Potteiger.
9. The proposed guardians have no interest adverse to that of their Mother.

WHEREFORE, Petitioners respectfully request that the Court, under Section 5511 of the
Probate, Estates and Fiduciaries Code, issue a citation to Doris Ann Potteiger, and her next of
kin, to show cause why Doris Ann Potteiger should not be adjudged to be an incapacitated person
and plenary guardian of her person and estate be appointed.
