ORDER OF COURT
AND NOW, this 1 ih day of July, 2006, based upon the evidence received,
this Court finds by clear and convincing evidence that John Johnson, is a totally
incapacitated person in need of plenary guardianship services.
The Court finds that Mr. Johnson, age 82, suffers from serious mental
illness, mental retardation and dementia. These conditions along with various
other health problems totally impair his capacity to receive and evaluate
information effectively and to make and communicate decisions regarding the
management of his physical health and safety.
Accordingly, Lisa Cook is appointed primary legal guardian of the person
of John Johnson. If Ms. Cook becomes unwilling, unable or otherwise
unavailable then Sue Kissinger is appointed as secondary legal guardian of his
person. The posting of bond in this matter is waived. The guardians are directed
to file reports in accordance with the provisions of the Probate, Estates and
Fiduciary Code applicable to such guardianships.
