PETITION FOR NAME CHANGE
COMES MINOR PETITIONER, Jesse Erin Salinas by and through her legal guardian(s) and
files this petition, and upon being duly sworn, respectfully represents and shows unto the Court:
1. That the Minor Petitioner is a minor and is a bona fide resident of the County of
Cumberland, Commonwealth of Pennsylvania, whose residence address is 500
Indiana Ave, Lemoyne, P A 17043. Minor Petitioner has been a bona fide resident
of Cumberland County, Commonwealth of Pennsylvania for 22 months
immediately prior to filing this Petition. Minor Petitioner was born on the 16th
day of February, 2001, in the County of Contra Costa, State of California.
2. Minor Petitioner's present name is Jesse Erin Salinas. This is the current name of
Minor Petitioner.
3. That the Minor Petitioner prays for the change of name for the following reasons:
(describe in detail): Mother married in 4/2005 and wishes minor to take family
name (stepfather's name) ofZampelli.
4. That the proposed change in the Minor Petitioner's name, if granted, will not be
detrimental to the interests of any other person and is not against the public
interest and is in the best interests of the child.
5. This Petition is not filed to defraud creditors.
