ELIZABETH D. SNOVER, being duly sworn according to law, upon her oath, deposes and says:
1. I am counsel for Holy Spirit Hospital in the above matter.
2. On the 18th day of July, 2006, I personally served upon Taryn Hench at Holy Spirit Hospital,
Camp Hill, Pennsylvania, and offered to read to her the Petition to adjudicate her to be incompetent and to
appoint a guardian for her person and her estate, the Citation issued pursuant thereto by the Clerk of the
Orphans' Court, and the Preliminary Decree scheduling a hearing.
