Jeff Taylor and Mt. Holly Auto Center, in the above-captioned matter, in the amount of Five
Thousand Nine Hundred Fifty-Nine and 02/100 ($5,959.02) Dollars, plus attorney's fees, interest,
and costs. Attached hereto as Exhibit "B" is an Affidavit including a certification from the Office
of the Prothonotary of Perry County.
