MECHANICS' LIEN CLAIM
1. Claimant, Masco Contracting Services, Inc. Ud/b/a American Aluminum &
Insulation Co. Ud/b/a Hungerford Insulation, is a business corporation with offices
located at 150 Fulling Mill Road, Middletown, Pennsylvania and files this mechanic's lien
claim as subcontractor.
2. The owner of the property subject to the lien is West Shore Evangelical
Free Church, 105 S. Market Street, Mechanicsburg, Pennsylvania.
3. The date on which claimant last supplied and installed materials for which
claim is made is April 5, 2006.
4. Claimant contracted with contractor, R.C. Skelly, Inc., and gave formal
notice of its intention to file this claim on May 31, 2006 by certified mail.
5. This claim is made for supplying and installing insulation in connection
with the below mentioned property.
6. The amount claim to be due is $4,500.00 plus interest.
