PETITION FOR CHANGE OF NAME
The petitioner, Jamie L. Black, by her counsel Friedman & King, P.C., John F. King,
Esquire, requests that this Court order a change of name for her natural daughter, Gabrielle Marie
Strausbaugh, a minor, pursuant to 54 Pa.C.S.A. 701 et seq., and in support thereof represents as
follows:
1. Petitioner resides in Cumberland County Pennsylvania, and has resided at the
following locations during the five years preceding the filing of this petition:
688 Market Street
Lemoyne, P A
(From March 2003 to present)
700 Castle Road, Lot 139
Manchester, P A 17345
(From September 2003 to March 2004)
205 Telegraph Road
Stafford, VA 22554
(from July 2001 to September 2003)
2, Petitioner is the natural mother of Gabrielle Marie Strausbaugh, a minor.

Gabrielle Marie Strausbaugh was born on July 21,2001, in York, Pennsylvania, as evidenced by
a copy of his birth certificate attached as Exhibit "A",
3. Petitioner believes that Carlos Valentin is the natural father of Gabrielle Marie
Strausbaugh, a minor, However, Mr. Valentin is not named as the father on the birth certificate.
In addition, Mr, Valentin has never paid support nor has paternity ever been legally detennined.
4. The alleged natural father has seen Gabrielle Marie Strausbaugh four or five times
since birth, and has not seen nor contacted the minor for more than two years preceding this
petition.
5. Petitioner married Kevin R. Black on June 24, 2006, as evidenced by a copy of the
marriage certificate, attached as Exhibit "B".
6. Prior to the marriage, since March 2004, Petitioner and Kevin R. Black resided
together and have a child, Garrett Black, who was born on March 30, 2005.
7. Kevin R. Black and Gabrielle Marie Strausbaugh have fonned an emotional bond
over the years and Kevin R. Black has acted in all steads and instances as father to Gabrielle
Marie Strausbaugh.
8. Petitioner and her husband would like Gabrielle Marie Strausbaugh to be known
as Gabrielle Marie Black, to promote family unity and both feel it is in the best interest of the
mmor.
WHEREFORE, petitioner respectfully requests that this Court enter an Order changing
the name of the minor from Gabrielle Marie Strausbaugh to Gabrielle Marie Black.

AND NOW, this 28th day of July, 2006, upon consideration of the attached
Petition for Change of Name, a hearing is scheduled for Monday, October 16, 2006, at
3:00 p.m., in Courtroom No.1, Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, ~6 (see 54 Pa. C.S. ~701 [Historical and
Statutory Notes] [Main Volume]), and 54 Pa. C,S. ~~701 et seq.
IT IS ORDERED and DIRECTED that the natural father is also to be given notice
of the hearing by first-class mail at his last known address.
