AMENDED PETITION FOR APPOINTMENT OF
PLENARY GUARDIANSHIP OF PERSON AND ESTATE
AND NOW comes Rose Hlavac ("Petitioner" or "Proposed Legal Guardian"), by and
through her attorney, Elizabeth A. Hoffman, to submit the following amendment to this petition:
1. Paragraphs 1 through 14 of the Petition for Appointment of Plenary Guardianship
of Person and Estate ("original Petition") are incorporated herein by reference thereto.
2. Paragraph 2 of the original Petition should be amended to state the following:
Nicole M. Hlavac, the alleged incapacitated person, was born on June 28, 1988, and is
temporarily residing at the Melmark home, 2600 Wayland Road, Berwyn, Pennsylvania 19312,
an educational institution for mentally impaired persons. Her permanent or legal address
remains in Cumberland County where her mother, Rose Hlavac, resides.
