1. Claimant, Masco Contracting Services, Inc. Udlb/a American Aluminum &
Insulation Co. tJdlb/a Hungerford Insulation, is a business corporation with offices
located at 150 Fulling Mill Road, Middletown, Pennsylvania and files this mechanic's lien
claim as subcontractor.
2. The owner of the property subject to the lien is Slate Hill Mennonite
Church, 1352 Slate Hill Road, Shiremanstown, Pennsylvania.
3. The date on which claimant last supplied and installed materials for which
claim is made is March 31, 2006,
4. Claimant contracted with contractor, R.C. Skelly, Inc., and gave formal
notice of its intention to file this claim on June 23, 2006 by attaching the notice to
several conspicuous places on the building.
5. This claim is made for supplying and installing insulation in connection
with the below mentioned property.
6, The amount claim to be due is $2,595.00 plus interest.

1. I am an adult and am a salesman for Masco Contracting Services Inc.

2. At 9:30 a.m. on August 4, 2006, pursuant to the authority granted in me
by Masco Contracting Services Inc. tldlb/a American Aluminum & Insulation Co. t1dlbla
Hungerford Insulation, I duly served upon Slate Hill Mennonite Church a Notice of
Mechanic's Lien Claim, by personally attaching a copy of the notice to a conspicuous
part of the Improvement at 1352 Slate Hill Road, Shiremanstown, Pennsylvania
pursuant to 49 P.S. 1501(d). A true and correct copy of the said notice is attached
hereto.
