COMES PETmONER, Lori Ellen Oommen by this petition, and upon being duly sworn,
respectfully represents and shows unto the Court:
1. That the Petitioner is of full legal age and is a bona fide resident of the County of
Cumberland, Commonwealth of Pennsylvania, whose residence address is 1114
Granada Lane, Mechanicsburg, P A 17055. Petitioner has been a bona fide resident
of Cumberland County, State of Pennsylvania for seventeen years immediately prior
to filing this Petition. Petitioner was born on the 19th day of March, 1965, in the
County of Passaic, State of New Jersey, Country of USA.
2. Petitioner's present name is Lori Ellen Oommen. This is the current name of
Petitioner on her social security card and all other records.

Petition To Change Name in the captioned matter, and upon presentation of proof of
publication of notice as required by law, together with proof that there are no judgments
or decrees or criminal convictions of record or any other matter of I:ike character against
the Petitioner, Lori Ellen Oommen, and it appearing that there is no lawful objection to
the request to change the name of the Petitioner, and after hearing held on November
