COMPLAINT
AND NOW, 26*h day of April, 2002, comes Plaintiff, through its undersigned attorneys, upon a cause
of action whereof the following is a statement:
1. Plaintiff is West Shore Emergency Medical Services, a Pennsylvania corporation organized
and existing under the laws of the Commonwealth of Pennsylvania with is principal office located at 503
North 21st Street, Camp Hill, Cumberland County, Pennsylvania 17011-2204 and is a successor by merger
to Chambersburg Area ALS.
2. Defendant is Perini Services, Inc./Southampton Manor Limited Partnership trading as
Shippensburg Health Care Center, with its principal place of business located at 127 Walnut Bottom Road
Shippensburg, Cumberland County, Pennsylvania.

Defendant is a skilled nursing facility that regularly requires the transport of its residents off
4. Chambersburg Area ALS was, prior to its merger with West Shore Emergency Medical
Services, a provider of such off site transportation services for residents of Defendant.
5. Subsequent to the merger of Chambersburg Area ALS with West Shore Emergency Medical
Services, West Shore Emergency Medical Services was regularly the provider of such off site transportation
services to Defendant.
6. At Defendant's special instance and request, Chambersburg Area ALS provided transport
services for Defendant's residents for the call dates, call numbers and for the charges reflected on West
Shore Emergency Medical Services' records, true and correct copies of the relevant portions of which are
attached hereto, made a part hereof and marked as Exhibit "A".
7. At Defendant's special instance and request, West Shore Emergency Medical Services
3rovided transport services for Defendant's residents for the call dates, call numbers and for the charges
reflected on West Shore Emergency Medical Services' records, true and correct copies of the relevant
portions of which are attached hereto, made a part hereof and marked as Exhibit "B".
8. The patient numbers and patient names of Defendant's residents have been redacted from
Exhibits "A" and "B" hereto to protect the resident's confidentiality, but are known to Defendant.

Defendant ordered, received and accepted the patient transports reflected on Exhibits "A" and
10. The charges set forth on Exhibits "A" and "B" hereto were fair and reasonable charges at the
time and the charges which Defendant agreed to pay.
-11. All credits, if any, to which Defendant is entitled are set forth on Exhibits "A" and "B" hereto.
12. Although a demand has been made, Defendant has failed to make payment of the amounts
due to Plaintiff as set forth on Exhibits "A" and "B" hereto to wit:
Exhibit "A" $26,489.78
Exhibit "B" $18.782.50
TOTAL $45,272.28
WHEREFORE, Plaintiff demands judgment against Defendant in the amount of $45,272.28 plus
nterest and costs of suit.
