For dates of death on or after July 1. 1994 and before January 1, 1995, the tax rate imposed on the net value of transfers to or for the use of the surviving spouse is 3%
[72 PS 89916 (a) (1.1) (i)],
For dates of death on or after January 1, 1995, the tax rate imposed on the net value of transfers to or for the use of the surviving spouse is 0% [72 PS 89116 (a) (1.1) (ii)]
The statute does not exempt a transfer to a surviving spouse from tax, and the statutory requirements for disclosure of assets and filing a tax return are still applicable even if
the surviving spouse is the only beneficiary.
For dates of death on or after July 1, 2000:
The tax rate imposed on the net value of transfers from a deceased child twenty-one years of age or younger at death to or for the use of a natural parent, an adoptive parent,
or a stepparent of the child is 0% [72 P.S 89116(a)(12)].
The tax rate imposed on the net value of transfers to or for the use of the decedent's lineal beneficiaries is 4.5%, except as noted in 7'2 P.S, 8 9116(1 .2) [72 P,S, 89116(a)(1 )].
The tax rate imposed on the net value of transfers to or for the use of the decedent's siblings is 12% (72 P.S. 8 9116(a)(1.3)]. A sibling is defined, under Section 9102. as an
individual who has at least one parent in common with the decedent, whether by blood or adoption.

1, JOYCE U. ADAMS, an unremarried widow, currently of Lower Allen Towship,
Cumberland County, Pennsylvania being of sound and disposing mind" memory and understanding, do
make, publish and declare this my Last Will and Testament, hereby revoking and making void any
and all prior Wills by me at any time heretofore made.

I direct the payment of all my just debts and funeral expenses as soon after my decease as the
same ean conveniently be done.

There is currently in existence a Funded Revocable Trust Agreement made byme, my
late husband, and my son Larry E. Adams, with Dauphin Deposit Bank and Trust company for the
benefit of my said son, dated February 25, 1992. Dauphin Deposit Bank and Trust Company has,
through various takeovers and mergers now been succeeded by Manufacturers and Traders Trust
Company, more commonly called M & T Bank. It is currently being considered as to whether or
not this Trust needs to be revoked in an effort to order my affairs in a bctter and more productive
fashion. In the event this Trust is revoked or amended or in the event before the date of my
death a new and similar trust is established, I hereby give, devise, and bequeath all the rest, residue
and remainder of my estate, real, personal and mixed, whatsoever and wheresoever situate to the
Trustee of such Trust or Trusts, as the case may be. I am currently contemplating naming PNC
Bank, National Association as such Trustee, but this grant and gift shall be considered to go to any
Trustee or Successor Trustee of such Trust or Trusts, as the case may be. My Trustee shall add
the property given, bequeathed and devised by this Item to the principal of such Trust or Trusts,
as the case may be, and shall hold, administer and distribute the property in accordance with the
provisions ofthe said Trust or Trusts, as the case may be

I nominate, constitute and appoint my son, LARRY E. ADAMS and PNC BANK,
NATIONAL ASSOCIA TION, to be the Co-Executors of this my Last Will and Testament. I
further direct that they shall not be required to file bond or other security in the Office of the
Register of Wills for the purpose of administering my Estate.

Estimated indicated value can also be determined by using the Gross Adjustment of sale price for each
comparable (comp) as a measure of relative quality of the comp, A lower adjustment indicates a beller comp, and
vice versa. The ratio of gross dollar adjustment to sale price for each of the comps is used to calculate the weight
each comp should have in a weighted calculation. This weighted calculation is used to also indicate the estimated
value of the subject As with any method this technique is not perfect. However it does do a very goOd job of
giving more weight to the most similar comps while at the same time minimizing values near the extremes of the
indicated value range. The estimated weighted value is: $108,900.
The definition of Market Value is from regulations published by federal regulatory agencies pursuant to Title XI of

THIS TRUST AGREEMENT is entered III to by and between JOYCE U. ADAMS,
unremarned widow, currently of the TO\\I1ship of Lower Allen, of the County of Cumberbnd,
Commonwealth of Pennsylvania, hereinafter called "Trustor,"and PNC BANK, National
Association, hereinafter referred to as "Trustee"
Reference in this Trust to the "Trustee" shall be deemed a reference to originally named
Trustee or to whomever is serving as Trustee or Co-Trustees, whether original, alternate, 0 r
successor.
The initial primary Beneficiaries of this Trust Estate shall be JOYCE U. ADAMS
The effective date of this Trust shall be the date of signing of this Trust Agreement.
I. TRUST PROPERTY
A. Original Trust Property
The Trustor grants, assigns, and sees, over to trustee and its successors, all the
property described in Schedule "A", annexed hereto and said property shall be
considered the original corpus of the Trust Estate.
II. PAYMENTS OF INCOME AND PRINCIPAL
A. During the lifetime ofthe Trustor, the Trustee shall pay all net income
to her in quarterly or other convenient installments. If in the discretion of
the trustee the Trustor is not capable of managing her own affairs, the
income of the trust may be used in the Trustee's discretion for the health,
maintenance and support of the Trustor. Income: not expanded may be added
to the principal of the Trust, in the Trustee's discretion.
B. Trustee may payor apply principal for the benefit of the Trustor in the
Trustee's discretion for her health, maintenance and support. Principal may
be paid directly to the Trustor or applied for the Trustor's benefit, in the
discretion of the Trustee.
C. At the death of th e Trustor, the undistributed principal and income of the
Trust shall be paid directly over to the Executor of the Estate of the Trustor,
and this trust shall cease assignment by a beneficiary, or available to anyone
having a claim against a beneficiary, before actual payment to the
beneficiary .
III. I direct that the interests of all beneficiaries in the Trust hereby created,
whether in the principal or income thereof, shall be free from liability to attachment
or other legal process issued at the instance of any creditor or assignee of such
beneficiary, and I direct that no payment shall be made by way of anticipation of
sums which may thereafter accrue to any beneficiary.
IV. TRUSTEE"S POWERS:
In addition to the powers granted to Trustee by law, Trustee shall have the power to
retain any property it shall receive hereunder; to invest as it sees fit in all forms of
property including mutual fund operated by Trustee or an affiliate of the Trustee; to
make distributions in cash or kind; to borrow or pledge assets at my request; to settle
disputes; to exchange or sell real or personal property publicly or privately for such
prices as it deems proper and to hold investments in nominee name.
V.
DEATH TAXES:
Unless my Will provides otherwise, any death taxes on my estate, including interest
and penalties, attributable to the principal of this trust shall be paid from that portion
of such principal subject to the particular tax.
VI. ADDITIONS:
With the approval of Trustee, property may be added to this trust.
VII. REVOCATION & AMENDMENT:
I may amend or revoke this Agreement by letter delivered to the Trustee during my
lifetime. Trustee may resign by giving me 30 days written notice. If Trustee decides
that it is impractical to administer this trust, it may pay the fund to me, or to any
person or persons entitled to the income or to have the income applied for their
benefit.
VIII. FEES:
Trustee shall receive compensation at its schedule of rates in effect from time to
time. When services are rendered to the trust.
IX. LAW:
The situs of the trust is Cumberland County, Pennsylvania and it shall be governed by
Pennsylvania law.
X: CONCERNING SELF-DEALING ET AL:
No rule of law against self-dealing, divided loyalty, or conflict of interest shall be
applied to render any transaction effected by the fiduciaries void, voidable, or
otherwise subject to attack solely for violation of such rule, nor shall the fiduciaries
incur any liability, nor shall any fiduciary commissions for acting hereunder be
reduced, solely for violation of such rule. Any transaction which involves self-
dealing, divided loyalty, or conflict of interest by the fiduciaries shall be judged by the
rules of law which would apply to the same transaction at arm's length between
strangers free of any element of self-dealing, divided loyalty, or conflict of interest.

1992, between ALVIN A. ADAMS, JOYCE U. ADAMS, his wife, LARRY E. ADAMS, their
son, of Camp Hill, Cumberland County, Pennsylvania (hereinafter called Settlors) and DAUPHIN
DEPOSIT BANK AND TRUST COMPANY (hereinafter called Trustee).
WITNESSEfH:
FIRST: Settlors grant, assign, and set over to Trustee and its successors all the property
desclibed in Schedule A annexed hereto and said property, together with all other property, real or
personal, that may be added to the trust (such property and additions being hereinafter called
principal), shall be held by Trustee, IN TRUST, upon the following terms:
SECOND: Trustee shall manage all property comprising the principal of this trust and shall
collectthc income thercCrom and accretions thereto and shall pay the net income and hold and
dIstribute the principal of said trust in the manner and fOITn provided for below and shall have the
following duties and responsibilities by way of itemiwtion which is not meant to be lIor should it
be considered to be a restnctiun of any kind on those inherent and customary dutlcs and
res(Xlllsibilities of trustees:
A. Trustec shall pay monthly bills for Lany Adams for: electricity,
tel cphone, water. scwer and rcruse, cable TV, fuel, i nsurancc and othcr Ii kc
things as they come due for Larry's residential premises.
B. Trustee shall pay other recurring bills for Liul). Adams from time to
tIme as they also come due, for example, personal and property taxes and the
like; and Trustee sball further hereby be enabled to invade principJI JS is necded
to maintain the residential premises.
C. Additionally, any income ovcr and alxlVe that required for the
alxwc-mcnlioneu periodiC payments emd the likc, as \Veil as so much of the
principal as is needed accOlding to the discretion of the Trustee, may bc used
and expended for any additional cosls of comfml, SUP[XH"( and maintenance
for Larry Adams, including medIcal, surgical and hospital cue, and any fonn
of further education Of vocational-technical training or the ltke which would enhance
Larry's capacity to take care of himself or possibly lead to a higher paying
job or position.
D. Additionally, should it be necessary, in the discretion of the
Trustee, Trustee may invade principal for any of the above:-mentioned purposes
and the like.
THIRD: Should Larry Adams have predeceased either or both of his parents, then they, or
the survivor of them, shall be considered the beneficiaries of this trust for and during their natural
lives, and the Trustee shall continue to manage all the property comprising this trust and shall
collect the income therefrom and accretions thereto and shall pay the net income to Larry's parents,
or the survivor of them, for and during their natural lives in such pericxlic installments as Trustee
shall find convenient, but at least as often as quarter-annually. Upon the death of the survivor of
Larry's parents, the balance remaining in the trust shall be distributed as is provided for below.
FOURTH: UlXlI1 the death of Larry, or the survivor of his parents, whichever comes last,
Trustee shall distlibute the remaining principal and any accumulated or undistributed income, as
follows:
A. One-third (1/3) thereof to the Grace United Methcxlist Church, 313 Herman
A venue, Lemoyne, Pennsylvania. This gift to be strictly for the use of its building fund and for
general maintenance of the buildings and grounds.
B. One-third (1/3) thereof to the Lincoln Chapel, L.aurclton, PA 17835-9998,
strictly for the use of its building fund and for general maintenance of the buildings ,md grounds.
C. One-third (1/3) thereof to Temple Beth Shalom of West Shore Jewish
Community, 913 Allemlale Road, Mcchanicsburg, Pennsylvania, strictly for the use 01 its building
lund and for general maintenance 01 the builJings anJ grounds.
Flf'iH: The principal and income of this trust shall be free from anticipation, ;Lssignment,
pledge or obligatiun of Settlors and shall not bc subJcct to any executiun or attachmcnl or to
\uluntary or involuntary alIenation.
SIXTI I: Trustee shall have the folIOl\'lng powers in addllionto those vested In it by law
and by othcr prOVisions of this trust, applicable to all properly, whether prinCIpal or income,
exelClsable withollt court approval, and efTcctlve until actual distribution of all properly:
A. To retain any or all of the assets of this trust, real or personal,
including stock of Trustee, without reg,u-d to any principle or diversification
or risk.

B. To invest in all fonus of property, including stock, common
trust funds and mortgage investment funds, without restriction to investments
authorized for Pennsylvania fiduciaries, as it deems proper, without regard
to any principle of diversification or risk.
C. To sell at public or private sale, to exchange, or to lease for any
period of time, any real or personal property and to give options for sales,
exchanges or leases, for such prices and upon such tenus or conditions as it
deems proper.
D. To allocate receipts and expenses to principal or income or partly
to each as Trustee from time to time thinks proper in its sole discretion.
E. To hold property in the name of Settlor, or in his name without
designation of any fiduciary capacity, or in the name of a nominee or
unregistered.
SEVENTH: Subject to the approval of Trustee, anyone may add property, real or
personal, to the principal of this trust by deed, will or otherwise.
EIGHTH: Trustee may receive reasonable compensation at prevailing rates for fiduciaries.
Such payments shall be made from time to time during the period over which iLs services are
performed.
NINTH: If at anytime hereafter, a proceeding is initiated in any jUlisdlction to have a
guardian appointed for Larry Adams, it is the express intention of the SettIors herein, that the court
making such a detennination, appoint Dauphin Deposit Bank and Trust Company, or its successor,
as the guardian of the eslate of Lan)' Adams.
TENTH: Selllars reserve to themselves the right, by the unanimous consent of those \\ho
lJlay then'be living, by an instrument in writing intended to take effect during the lifetimes of
Selllors or the survivors oj thelJl, signed by all the Settlors then liVing or their guanJians or
attollleys-in-fact should they be then incapacitated, and delivered to Trustee to revoke or amend
this Agreement in whole or in part. PlOviJed that the duties, lXl\\ers ami liabilities of Tlustec shall
nol be subslantially changed without his written consent.
ELEVENTH: The situs of this trust tor administrative and accounting purposes shall be in
the County of Cumberland and COIllmonwealth of Pennsylvania, and all quesl10ns pertallling to the
construction or validity of the provisions of this instrument shall be governed by the la\\s of that

order to initiate a claim on the contract, please submit the following requirements:
. AEL Death Claim Statement form completed by Larry Adams, the named
beneficiary. Please provide complete information and fill in all sections of the
enclosed form, paying particular attention to the Taxpayer Identification Number
and Certification section. Each beneficiary must complete an enclosed form.
Please note that the beneficiary signature will need to be notarized.
. One Certified Death Certificate with a raised or color-coded. court seal.
. IRS W-9 Taxpayer Identification Number and Certification Form completed by
you as beneficiary.
. The annuity contract, if available. If the contract has been lost or destroyed,
please note this on the AEL Death Claim Statement Form.
Please note: If you would like to have the original death certificate returned, please write
the name and address of the recipient on the back of the certificate. If this information is
not included, we cannot return the death certificate.
