Bogar, Esquire, and files the following municipal claim against JEREMY A. FORINO, an adult individual, owning property located at 14 Sinclair Road, Mechanicsburg, Monroe Township, Cumberland County, Pennsylvania, 17055, his last known mailing address being: 14 Sinclair Road, Mechanicsburg, Pennsylvania 17055. JEREMY A. FORINO is the owner of all that certain lot of ground, including any improvements thereon, being more fully described as follows: ALL THAT CERTAIN house and lot of ground situate in Monroe Township, Cumberland County, Pennsylvania, bounded and described as follows: BEGINNING at a point formed by the intersection of the centerline of Sinclair Road (formerly Williams Grove Road) and the southern line of Nixon Drive, a fifty (50) foot wide street, extending westwardly from Sinclair Road (formerly Williams Grove Road); thence along Sinclair Road (formerly Williams Grove Road) South four (4) degrees fifteen (15) minutes East ninety (90) feet to a point; thence South , eighty-five (85) degrees forty-seven (47) minutes West two hundred two and nineteen hundredths (202.19) feet to a point; thence North four (4) degrees thirteen (13) minutes West ninety (90) feet to a point; thence north eighty-five (85) degrees forty-seven (47) minutes West two hundred one and ninety-nine hundredths (201.99) feet to a point, the place of BEGINNING. HAVING THEREON ERECTED a single family dwelling known and numbered as 14 Sinclair Road, Mechanicsburg, Pennsylvania. BEING the same premises which Lois J. Wagner, single person, by her deed dated March 23, 2001 and recorded March 28, 2001 in the Cumberland County Recorder of Deeds Office in Deed Book 241, Page 702 granted and conveyed unto Jeremy A. Forino, married person. Tax Parcel No. 22-24-0783-119. Monroe Township Municipality Authority hereby files this Municipal Lien for Services and Sewer Rates levied and assessed from March 28, 2001 through and including June 30, 2006 (first quarter calendar year 2001 through and including second quarter calendar year 2006). Said Authority services and sewer rates were levied and assessed pursuant to Authority Resolution No, 02- 04, same being known as a Resolution of the Board of Monroe Township Municipal Authority imposing user charges to be collected from the owner of each improved property served or to be served by this Authority's sewer systems; and providing for payment and collection of such user charges, Attached hereto and incorporated herein are the statements forwarded by Monroe -2- , Township Municipal Authority to JEREMY A. FaRINO for sewerage due to be treated from March 28, 2001 through and including June 30, 2006 (first quarter calendar year 2001 through and including second quarter calendar year 2006), which services total is in the amount of $1,167,32, which sum was duly assessed against JEREMY A.
