PETITION TO MODIFY MARRIAGE RECORD AND CERTIFICATION
NOW, comes the Petitioners, Adam M. Lewis and Kimberly A. Myers, (hereinafter
"petitioners") by and through their attorney, Frederick Lester, Esquire, and r~ectfully g

1. Petitioners wishes to amend the Marriage Record and the Certificati0i5~~

Excerpts From Marriage License Record ("Certification") so that both doc~e!ijs ~

accurately reflect Kimberly A. Myers's proper date of birth. (Marriage Record and -oJ
Certification marked exhibit "A" and "B" respectively, attached and incorporated by
reference herein).
2. Petitioners' marriage license was issued on May 4,2006, and petitioners were
married on May 13,2006. Petitioners failed to inspect the Marriage Record or
Certification and, consequently, did not notice that petitioner, Kimberly A. Myers's date
of birth was incorrect. The Marriage Record and the Certification incorrectly list
petitioner Kimberly A. Myers's date of birth as December 4, 1979, when her actual date
of birth is December 3, 1979. (See petitioner's Commonwealth of Pennsylvania
Certification of Birth and valid Virginia driver's license marked as exhibits "C" and "D"
respectively, and incorporated by reference herein).

3. Petitioners wish that these documents accurately reflect all relevant information
contained within and wish to have both of these documents amended, and any internal
documents kept by the Register of Wills relative to their Marriage Record, if they exist.
WHEREFORE, for each of the following reasons, petitioner hereby moves this Court
to ORDER the Register of Wills to amend these documents as requested by petitioners.
