CHANGE OF NAME
PETITION TO CHANGE NAME
AND NOW comes Petitioner, Jagtar Singh, by and through his
attorney, Thomas D. Gould, and requests this Honorable Court,
pursuant to 54 P.S. Section 701, to issue a Decree changing the
name of Petitioner to JAGTAR BHADRA, based on the following:
1. Petitioner is an adult citizen who resides at 818
Ridgewood Drive, Mechanicsburg, Cumberland County, Pennsylvania.
2. Petitioner is 51 years of age having been born on March
15, 1955.
3. During the last five (5) years the Petitioner has resided
at the following location:
818 Ridgewood Drive, Mechanicsburg, PA
06/01 - present
4. Petitioner desires to change his last name to his
ancestry' village name of BHAURA to avoid confusion with others
named Singh.

5, Petitioner has no criminal charges, liens, judgments or
decrees of record, other than a mortgage on his residence, or any
other matter pending of like character against him in this or any
other jurisdiction.
6, The Petitioner is married to GURPREET KAUR who concerns
in this Petition and has two children, OPINDER SINGH and ANMOAL
SINGH.
WHEREFORE, Petitioner respectfully requests this Honorable
Court to set this matter for hearing and enter an Order directing
Petitioner to give Notice of the filing of this action and the date
and time set for Hearing, which shall not be less than one month or
more than three months after the filing of this Petition.
After hearing it is respectfully requested that this Honorable
Court issue a Decree changing the name of Petitioner from Jartar
Singh to Jagtar Bhaura.

ORDER OF COURT
AND NOW, this 21 st day of September, 2006, upon consideration of the attached
Petition To Change Name, a hearing is scheduled for Wednesday, December 20,2006, at
10:30 a.m., in Courtroom No.1, Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, ~6 (see 54 Pa. C.S. ~701 [Historical and
Statutory Notes] [Main Volume]), and 54 Pa. C.S. ~~701 et seq.
