OF JEAN L. FRALISH PURSUANT TO Pa. C.S.A. & 5521(c)
I, James L. Fralish, was appointed permanent and plenary guardian of the person and estate
by Decree of Kevin A. Hess, J., dated May 18,2006. This is my annual report for the period from
February 14, 2006, when funds of the incapacitated person were deposited to the escrow account of
Eric J. Wiener, Esquire, to June 13,2006, the date of death of Jean L. Fralish. Due to such death of
Jean L. Fralish on June 13, 2006, this is also my final report, and I aver that the principal balance on
hand shall be paid to the personal representative of the Estate of Jean L. Fralish.
Rq>ort of Guardian of the Person:
The permanent and plenary guardianship of the person of Jean L. Fralish was established on
May 18, 2006 and from that time until the time of her death on June 13,2006, she resided in the

Carlisle Regional Medical Center at Carlisle, Pennsylvania. Due to such death, n~er r~rt o~o ~
