Dear Judge Oler:
Pursuant to my telephone conversation earlier today with your secretary, I am writing to
formally request a continuance of the name-change hearing in the above-referenced matter. The
hearing is currently scheduled for August 1, 2002, at 1:30 p.m.; however, due to the fact that the
Notice was not published ten days prior to the hearing in accordance with Cumberland County Rule
of Civil Procedure 5 (3), I respectfully request that you continue the hearing in this matter to the next
earliest date.
Please notify me with the new date and time for the hearing. Should you have any questions
or should you wish to discuss this matter further, please do not hesitate to contact me.

PETITION FOR NAME CHANGE
AND NOW, this /6 ~ day of May, 2002, comes the Petitioner, Olive Bolinger Glass,
by and through her attorneys, Hanft & Knight, P.C., and files the following Petition, and in support
thereof, avers the following:
1. Petitioner, Olive Bolinger Glass, is an adult individual residing at 1220 Walnut
Bottom Road, Carlisle, Cumberland County, Pennsylvania.
2. Petitioner's date of birth is February 14, 1932.
3. Petitioner's maiden name was Olive Poleck. Petitioner was not given a middle name.
4. PetitionermarriedRonaldE. Boling~r in July, 1953. Petitionerwas thereafter known
as Olive Bolinger.
5. Petitioner's husband, Ronald E. Bolinger, passed away in 1973.
6. Petitioner cominued to be known as Olive Bolinger until she married her second
husband, Edward L. Glass, in 1983. Thereafter, Petitioner was known as Olive Bolinger Glass.
7. Petitioner's second husband, Edward L. Glass, passed away on July 6, 2000.
8. Petitioner has been a resident of Cumberland County, Pennsylvania for at least six
(6) months previous to the filing of this Petition.
9. Petitioner now desires to re-take the surname of her first husband, add a middle
name, and thereby change her name from Olive Bolinger Glass to Olive Ranea Bolinger.
10. Petitioner does not wish to change her name for any illegal purpose or to avoid any
obligation. Petitioner has been fingerprinted by the Pennsylvania State Police and her fingerprint
card is attached hereto as Exhibit "A" and by reference incorporated herein.
11. Petitioner's parents are no longer alive.
12. Petitioner's son still use the surname "Bolinger," and her daughter uses her married
name.
13. Petitioner, Olive Bolinger Glass's Social Security number is 205-26-4735.
WHEREFORE, Petitioner, Olive Bolinger Glass, respectfully requests this Honorable Court
to issue an Order changing her name from Olive Bolinger Glass to Olive Ranea Bolinger.
