MUNICIPAL LIEN FOR SEWER SERVICES
North Middleton Authority, North Middleton Township, County of Cumberland,
Commonwealth of Pennsylvania by its solicitor, Broujos & Gilroy, P.C., hereby files its claim
for water and sewer services against the real estate hereinafter described and sets forth its
claim as follows:
STATEMENT OF CLAIM
The name of the municipality by which this claim is filed is North Middleton Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name or names of owner or owners or reputed owner or owners, of the property against
which this claim is filed is Domingo Vazquez and Brenda Stone, 40 Bellaire Avenue, Carlisle,
Cumberland County, Pennsylvania.

A description of the property against which it is filed is attached hereto and marked Exhibit
"A", with an address of 40 Bellaire Avenue, Carlisle, Pennsylvania.

The authorities under and by virtue of which this lien is filed is the Municipal Claims and
Liens Act of May 16,1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid sewer user fees against the above described property in the
amount of $405.92.

Interest is claimed on the above-mentioned amount from this Ist day of May, 2006.

MADE THE 2nd day of July, in the year of our Lord two thousand four (2004).
BETWEEN CUMBERLAND VALLEY HABITAT FOR HUMANITY,
a Pennsylvania nonprofit corporation, hereinafter referred to as:
Grantor,
and DOMINGO VAZQUEZ and BRENDA STONE of Cumberland County, Pennsylvania,
hereinafter referred to as:
Grantees,
WITNESSETH, that in consideration of Fifty-three Thousand, Two Hundred Sixteen Dollars
and 00/100 ($53, 216.0)) in hand paid, the receipt whereof is hereby acknowledged, the said Grantor
does hereby grant and convey to the said Grantees, their heirs and assigns,
ALL that certain lot of ground situate in North Middleton Township, Cumberland County, Pennsylvania and
being Lot No. 39 as shown on Plan No. 2 of Hill Top Manor, dated May 27, 1964, and recorded March 31,
1967, in Cumberland County Plan Book 18, Page 77, as follows:
LOT No. 39: BEGINNING at a point on the northern line of Bellaire Avenue (33 feet wide) at the line
dividing Lots Nos. 38 and 39 as shown on the previously mentioned plan; thence by the northern line of
Bellaire Avenue by a curve to the left having a radius of 113 feet the are distance of 70.0 feet to the line
dividing Lots Nos. 39 and 40 as shown on said Plan; thence by said dividing line North 04 degrees 09
minutes 30 seconds East 162.27 feet to a point; thence South 68 degrees 05 minutes 30 seconds East
167.79 feet to a point; thence by the dividing line between Lots Nos. 38 and 39 South 39 degrees 39
minutes West 162.27 feet to the place of BEGINNING.
BEING part of the same premises which James E. Schlusser and Leighann Schlusser, husband and wife,
by deed dated September 5, 2002 and recorded in the Office of the Recorder of Deeds in and for
Cumberland County in Deed Book253 Page 30t 3 granted and conveyed unto Cumberland Valley Habitat
for Humanity, Grantor herein.

AND the said Grantor hereby covenants and agrees that it will warrant specially the property hereby
conveyed.
