Reason: I, Antoinette G. Gathers am hereby requesting that legal and physical custody
of James Gathers, my son, is placed forthwith to his grandparents Anthony L. and
Pamella M. Gathers. The reason for this request is to ensure that James Gathers will
have adequate health insurance as well as any and all necessities that may arise that I
am not currently unable to provide. In addition, I am currently trying to join one of the
United States Forces in which I may not enter if I have any dependents.

ORDER OF THE COURT
AND NOW, THIS ~ day of October 2006: IT IS ORDERED AND DIRECTED that
legal and physical custody of the minor child James Gathers shall be placed in the
custody of Anthony and Pamella Gathers.
Cumberland County Court of Common Pleas will retain jurisdiction over this matter.
This Order will remain in effect until further Order of Court.
