C.S.A. 701, et. seq., and in support thereof represents as follows: 1. Petitioner, Florence Pryor Barker is an adult individual currently residing at 3110 Columbia Avenue, Camp Hill, Pennsylvania and has resided at this address since 1990. 2. Petitioner, Florence Pryor Barker, was born in Dauphin County, Pennsylvania, on January 18, 1962. A copy of Petitioner's Certification of Birth is attached hereto, incorporated herein and marked as Exhibit "A". 3. Petitioner desires to change her name from Florence Pryor Barker to Lori Pryor Barker because she wishes to have her driver's license, birth certificate and all other legal documentation match her social security records, work history, credit and educational history which show as Lori Pryor Barker, the name she is called by all family and friends, and the first name she has been called since birth. 4. There are no minor children who will be affected by this Petition. 5. There are no Judgments, Liens or Decrees of like character of record against the Petitioner in Cumberland County, Pennsylvania. A copy of a judgment/lien search and a support lien inquiry is attached hereto, incorporated herein and marked as Exhibit "B". 6. Petitioner has presented herself for finger-printing at the Camp Hill Borough Police Department on August 21,2006. A copy of Petitioner's fmger-printing is attached hereto, incorporated herein and marked as Exhibit "C".
