RE: Raymond B. Cummings
Dear Ms. Whitman:
On Monday October 2) 2006 I was called in as a Consultant to the Regional
Medical Center in Carlisle, to evaluate the mental status of Raymond B.
Cummings. I have been in private practice in the area since 1986 and serve the
hospital as a member of the Medical Department. During the past ten years) I
have evaluated hundreds of geriatric cases in Skilled Nursing Homes as well as
the Center.
After reviewing the case charts. both present and past, interviewing the nursing
staff assigned to the case and meeting with the patient I concluded that Mr.
Cummings is impaired to the extent of not being able to manage his finances
and to make decisions in the best interest of his personal health and safety. He
requires the help of a guardian and a living arrangement such as a skilled
nursing facility would provide. Signs of Dementia are present He shows
memory loss and confusion. Alcohol abuse has resulted in loss of mental
capacity. If allowed to return to his current abode he would tend to use alcohol
excessively, and if found alive, would be taken to the emergency room for help.
It is also conceivable that his behaviors could result in a fire that might harm
others. The hospital has been able to restore partial stability but is not able to
provide an environment that he requires to meet his continuing basic needs.
He lacks the capacity to provide basic care to meet his minimum survival
requirements.

Deat Ms. Whitman:
As per yow: request. I retumed The Carlisle Regiottal Medical to test Mr.
Cummings. A W AIS III intelligent teSt was employed and his IQ was found to
be Borderline V62.89. His full scale score was found to be 74. He indica.ted to
me that when he 'Was young hi..; intelligence score was on the opposite end of
the scale; inclicated by recall to be 120.
In testing him, the thought processes would wander at times never completing
the thought and going mto some non related :responses while not being aware
of h1S statanents. Also) his math skills were found to be at the: second grade
level He has bencfrted from his stay in the hospita1and would continue to
benefit in an assisted living. nursing home or group home. Left on his own, I
believe he would revert to excessive alcohol consumption.
Thetefore, he can not manage I#s finances in a manner that would be in his
best interest for health and safety. Although the Dementia is mild it interferes
with his decision making ability. In brief encountets he comes across as if he
has his faculties together, but press on a.nd one begins to see his problems.
Guardianship appears to be necessuy if his needs are to be met.
