the estate of the above decedent.
Decedent was domiciled at death in Upper Miftlin Township, Cumberland County,
Pennsylvania, with his last family or principal residence at 1446 Mountain Road, Newburg,
Pennsylvania.
Decedent, then 32 years of age, died on August 15,2006 at US Rt. 11 at Smith Road, West
Pennsboro Township, Pennsylvania.
