Weller and Patricia K. Weller, husband and wife, who, through their undersigned counsel, Jerry A. Weigle, Esquire, request the Court to order a change of name for said minor child pursuant to 54 Pa. Com. Stat. Am. g701 et seq. based upon the following: I. Seth Michael Baltimore is a resident of Cumberland County, Pennsylvania, and has resided at 117 South Prince Street, Shippensburg, Pennsylvania 17257, with his maternal grandparents, William R. Weller and Patricia K. Weller, husband and wife, continuously for the last five (5) years and since 1993. 2. Seth Michael Baltimore was born December 19, 1989, in Franklin County, Pennsylvania, to Julian Troy Baltimore and Stephanie Weller. 3. A birth certificate was first issued by the Vital Records Division of the Pennsylvania Department of Health in the name of "Baby Boy Weller" but was subsequently amended to the name of Seth Michael Baltimore. A copy of the current birth certificate is attached hereto, made a part hereof and marked "Petitioner's Exhibit #1." 4. A Federal Social Security Number was issued to the Petitioners at birth in the name of Seth Michael Weller. A copy of Petitioner's Social Security card bearing a Social Security Number of 159-72-7312 is attached hereto, made a part hereof and marked "Petitioner's Exhibit #2." 5. Your Petitioner requests that his name be changed from Seth Michael Baltimore to Seth Michael Weller for the following reasons: WEIGLE & ASSOCIATES. Weller in school, in church and in the community where he lives practically since birth and wishes to continue to be known throughout his life as Seth Michael Weller. B. Seth Michael Baltimore has been raised in the home of his maternal grandparents, William R. Weller and Patricia K. Weller, for almost all of his sixteen (16) years and should bear the name "Weller" for all legal purposes. C. Seth Michael Baltimore is presently unable to obtain a driver's license in the , Commonwealth of Pennsylvania with inconsistent names on his birth certificate and Social Security card. D. Having inconsistent names on vital pieces of identification could well cause the Petitioner to have legal difficulties and to suffer lost opportunities throughout his life. E. Petitioner wishes any future descendants to bear the name "Weller" which is the name he has been known by practically since birth. 6. There are no judgments, orders or decrees of record in any jurisdiction or any other matter oflike character against Seth Michael Baltimore. 7. Both natural parents of Seth Michael Baltimore consent to the request of their son for a change of surname pursuant to this Petition. Written consents of both parents are attached hereto, made a part hereof and marked "Petitioner's Exhibit 3." 8. Your Petitioner requests that the Court appoint his maternal grandparents, William R. Weller and Patricia K. Weller, as his legal guardian for the limited purpose of securing for him the name change requested in this Petition, pursuant to Pa. Rule of Civil Procedure 2031 (g).
