On April 28, 2006, Alice I. Hoc~ died at the Sarah Todd Home in Carlisle. Her daughter,
Beverly A. Kelchner, and her son, Ronald C. Hoch, are the lineal descendants of Alice I.
Hoch.
In September 2006, Beverly A. Kelchner received a Information Notice and Taxpayer
Response Form from the Pa. Department of Revenue. The form referenced ajoint
checking bank account at the Orrstown Bank and the acoount was established to pay
medical bills for Alice 1. Hoch. The account contained $827.88.Unfortunately the form
has been misplaced. In good faith and rather than wait for another form, I include a
reproduced form which calculates the tax at 4 ~ percentage and a check for $6.18 to pay
the tax.
