Voltz, Jr DOB: August 27,1986 Dr. Attorney Maffett: James Voltz has been a patient of mine since February 13, 2004. He is diagnosed with Autism and Bipolar Disorder. The extent of his disorders impairs his capacity to make and communicate decisions. He would be totally unable to make decisions effectively with regard to his financial resources or physical or mental well being. James is unable to receive and evaluate information and communicate decisions impaired to such a significant extent that he cannot meet the essential requirements for his physical health and safety. James is currently living successfully with his parents. He seems to be appropriately connected with essential services at the current time that. James is not capable of providing the required consents and approvals necessary for medical, psychological, and psychiatric treatment. He also doesn't have the capacity to make and communicate decisions necessary to give advance directives such as durable powers of attorney or trusts. In my professional opinion within a reasonable degree of medical certainty, I believe that James is an incapacitated person. Sincerely, l~O~"~.
