FINAL DECREE
AND NOW, this Z. Go - day of Oc.k b.c.-- , 2006, upon consideration of the
Petition for Adjudication of Incapacity and Appointment of a Guardian Over the Person of
Sheldon Grannison, and after a hearing on the matter, this Court hereby ORDERS AND
DECREES that:
1. Sheldon Grannison is adjudged an incapacitated person and that Neighborhood
Services is appointed plenary guardian over his person. The duties of the Plenary Guardian over
the Person shall be performed in accordance with the provisions of20 Pa. C.S.A. ~ 5501 et seq.
and as limited therein.
a. The Guardian of the Person shall file a Report on the social, medical and
other relevant conditions at least once during the first year of appointment
and annually thereafter as required by Pa. C.S.A. ~ 5521(c).
b. The Guardian of the Person shall have the authority to decide where
Sheldon Grannison shall live and how meals, personal care, transportation

and recreation shall be provided. The Guardian of the Person shall also

surgical procedures necessary for the well-being of Sheldon Grannison.
adjudication of capacity, the guardian shall file a Final Report with the
Court pursuant to 20 Pa. C.S.A. ~ 5521(c)(2).

d. Within seven (7) days of the date of this Final Decree, the guardian
shall make application on Sheldon Grannison's behalf for enrollment
in the Medical Assistance program or to obtain any other benefits
available to him.
2. The Guardian shall be paid a monthly fee of no more than $100.00 for services
rendered hereunder, which is to be payable from Estate income, and the Guardian may petition
the Court for grant of allowance for payment of other fees, costs or expenses from the Estate.
3. The Guardian shall serve upon and read to Sheldon Grannison this Decree, and
advise him of his right to file exceptions to this Decree within twenty (20) days of the date
herein, of his right to appeal to the Superior Court within thirty (30) days of the denial of the
exceptions, and of his right to petition the Court at any time to modify or terminate the plenary
guardianship ifthere is a significant change in his capacity or ifhis plenary guardian fails to
perform its duties in accordance with the Court's Decree.
