This Court hereby finds by clear and convincing evidence that: :' ~;j ~

1. Sheldon Grannison is 91 years of age, who resides at Blue Rid~ Vi est
Chateau, a personal care home, located at 10 House A venue, Camp Hill, Pennsylvania
17011.
2. Mr. Grannison is a domiciliary of Cumberland County, Pennsylvania.
3. Sheldon Grannison was personally served with the Petition, Citation and
Preliminary Decree on September 20,2006.
4. Sheldon Grannison suffers from Alzheimer's dementia, which renders him
unable to make and communicate decisions regarding his medical treatment and personal
or financial affairs.
. 5. Because of his mental condition, Sheldon Grannison's ability to receive
and evaluate information effectively is so impaired that he is totally unable to meet
essential requirements for his physical health and safety.
6. Sheldon Grannison's mental condition is not expected to improve, his
prognosIs IS very poor.
7. Sheldon Grannison has no living family members to assist him in making
decisions regarding his physical health and well being. Sheldon Grannison has not

executed a living will, advance directive for health care, power of attorney or other
document regarding his wishes pertaining to his medical care in the event of his
incapacity.
8. Sheldon Grannison is in need of guardianship services to assist him in
making decisions regarding his personal health and well-being.
9. Neighborhood Services, a non-profit corporation, has consented to serve
as guardian of the person of Sheldon Grannison.
10. Neighborhood Services is best suited to meet the needs of Sheldon
Grannison as an incapacitated person.
CONCLUSIONS OF LAW
1. Proper notice of the hearing held on October 26, 2006, was given to
Sheldon Grannison pursuant to 20 Pa. C.S.A. ~ 5511(a).
2. The Court of Common Pleas of Cumberland County has the authority to
appoint Neighborhood Services as guardian over Sheldon Grannison's person because he
is a resident of Cumberland County and is domiciled in Cumberland County.
3. Based upon the Findings of Fact, Sheldon Grannison is incapacitated and
Neighborhood Services is the appropriate guardian of the person of Sheldon Grannison.
