Irwin, III, Esquire, and presents this petition for change of name, representing as follows: 1. Petitioner is DAWNIE ROSE BENJAMIN, an adult individual residing at 21 Morgan Drive, Carlisle, Cumberland County, Pennsylvania 17015. 2. The petitioner was born on March 5, 1968 in St. Albans, Queens, New York. A copy of petitioner's birth certificate is incorporated herein by reference and attached hereto as Exhibit "A". 3. In or about 1977, while petitioner was in fourth grade, petitioner's parents began calling her "Tayari" instead of "Dawnie" and the following year obtained a social security card for her using the name of ''Tayari Rose Benjamin". A copy of petitioner's social security card is incorporated herein by reference and attached hereto as Exhibit "B". 4. Petitioner's driver's license has been in the name of "Tayari Rose Benjamin" since about 1988. A copy of her current driver's license is incorporated herein by reference and attached hereto as Exhibit "C". 5. Petitioner's diploma from Towson University, dated May 23,2000, is in the name of "Tayari Rose Benjamin". A copy of petitioner's college diploma is incorporated herein by reference and attached hereto as Exhibit "0". 6. There are no judgments or decrees of like character of record against the petitioner. 7. Petitioner desires a change of her name from "Dawnie Rose Benjamin" to "Tayari-Dawnie Rose Benjamin" in order to conform the public records and her formal name to the name which she has used for most of her life and by which she is known to all of her family, friends and associates. WHEREFORE, petitioner requests your Honorable Court, pursuant to Act of December 16,1982, P.L. 1309, No. 295, Section 2,54 Pa. Cons. Stat. Section 701, et seq., to do the following: A. Fix a hearing on this petition; and B. Enter a decree changing the name of petitioner from "Dawnie Rose Benjamin" to "Tayari-Dawnie Rose Benj HAROLD S.
