1. Petitioner is JAMES MICHAEL FISCHER, JR., who resides at 2117
Princeton Avenue, Apartment #6, Camp Hill, Cumberland County, Pennsylvania.
2. Petitioner has resided at the following addresses for the five years
previous to filing of this petition:
June, 2004 - Present: 2117 Princeton Avenue, Apartment #6, Camp Hill,
Cumberland County, Pennsylvania
March, 2001 - June, 2004 109 Bethel Street, Columbia, Lancaster County,
Pennsylvania
3. Petitioner desires and intends to change his name to RACHEL ANN
FISCHER.
4. Petitioner provides the following reason for this request: Petitioner is a
transitioning transsexual and is irretrievably committed to living as a person of the
opposite sex.

5. Petitioner states that no credit has been extended to him in the last ten
years under the name of RACHEL ANN FISCHER, and that this change of name is not
requested for the purpose of concealment of identity or for any fraudulent purpose.
WHEREFORE, Petitioner respectfully requests that this Honorable Court enter a
decree changing his name from JAMES MICHAEL FISCHER, JR. to RACHEL ANN
FISCHER.

Petition To Change Name, a hearing is scheduled for Thursday, February 22, 2007, at
3:45 p.m., in Courtroom No.1, Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioner shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, PoL. 1309, ~6 (see 54 Pa. C.S. ~701 [Historical and
