pro se files the following Petition for Change of Name:

Petitioner is a resident of Cumberland County, Pennsylvania and has resided at the
following address for the past three years:

New Cumberland, PA 17070
And two years prior at:
821 Bossier Avenue
Lemoyne, PA 17043
And four years prior at:
131 Brian Drive
Enola, PA 17025
Petitioner desires to have her name changed to VICTORIA MARIE ZUNA.
Petitioner desires to have her name changed due to a personal choice arising out of her
being generally known by the name she desires to take.
Petitioner is a citizen of the United States of America, having been born in the County of
Cumberland, Pennsylvania on November 7, 1967.
Petitioner is a single adult individualwho is the age of Thirty-Four (34) years.
WHEREFORE, petitioner respectfully requests your Honorable Court to enter an Order
of Court changing her name from Vickie Marie Zuna to VICTORIA MARIE ZUNA.
