DECREE
AND NOW, this IJ~y of ~~~ 2006, upon consideration of the Petition For
Appointment of a Permanent Guardian of the Person and Estate in accordance with 20 Pa.C.S.A.
5501 et seq., for JAMES N. VOLTZ, JR., the consents from the presumptive heirs attached
thereto; and, after a hearing held following due notice, IT IS HEREBY ORDERED AND
DECREED:
1. The ability to receive and evaluate infonnation effectively of James N. Voltz, Jr., the
alleged incapacitated person, is impaired because he suffers from Autism and Bipolar Disorder.
2. The ability to communicate decisions of James N. Voltz, Jr., the alleged incapacitated
person, is significantly impaired because of the aforesaid conditions.
3. The said James N. Voltz, Jr., is hereby adjudged an incapacitated person.
4. Kathryn A. Voltz and James N. Voltz, Sf. are hereby appointed permanent and
plenary guardian of the PERSON. The said permanent and plenary guardians of the PERSON
shall have the following powers and duties:

(a) to provide for the general care, maintenance and custody of James N~~
Voltz, Jr., the incapacitated person, including the ability to contract with anni ~ Q

(c) to sign and consent on behalf of James N. Voltz, Jr., the incapacitated
person, for the provision of medical procedures and services for his health,
welfare, and benefit; and,
(d) to provide those services necessary and proper for the health, support and
maintenance of James N. Voltz, Jr., the incapacitated person.
5. Kathryn A. Voltz and James N. Voltz, Sr. are hereby appointed permanent and plenary
guardians of the ESTATE. The said pennanent and plenary guardians of the ESTATE shall have
the following powers and duties:
(a) to obtain limited access to the financial accounts and related financial
information of James N. Voltz, Jr., incapacitated person, in order to transact
business and pay his bills and other liabilities;
(b) to sign checks, drafts and related financial instruments and documents in
order to pay bills, liabilities and related obligations of James N. Voltz, Jr.,
incapacitated person, for daily living and normal life activities, including the
provision of medical care; and,
(c) to provide those services necessary and proper to handle and maintain the
current financial affairs and status of James N. Voltz., incapacitated person.
This Court retains jurisdiction over any subsequent proceedings initiated by the parties in
this matter, including but not limited to, any and all accountings and reports required and filed by
the guardians.
