Taylor An alleged incapacitated person No. 21-06-0435 ORDER OF COURT AND NOW, this 20th day of November, 2006, IT IS HEREBY ORDERED AND DIRECTED that: 1. Unless circumstances dictate otherwise as determined by the Guardian, the Taylors will be allowed to dine out once weekly conditioned upon compliance with this Court's previous Orders and the protocols established by the Guardian. All hosts shall attempt to comply with the return time of 7:30 p.m., however, the hosts will be given some flexibility in regard to return time but said return time shall never be later than 8:00 p.m. 2. Any host desiring to take the Taylors out to dine pursuant to this Order must provide the Guardian with at least 48 hours advance notice seeking the 'Guardian's approval. 3. No host seeking permission to take the Taylors on an out-of-facility excursion will discuss the matter with the Taylors prior to receiving approval from the Guardian. 4. Any host taking the Taylors on an out-of-the-facility excursion must comply with the attached "Protocols for Dining Out". 6. No host taking the Taylors on an out-of-the-facility excursion will be permitted to take the Taylors to their former residence at 510 Appalachian Avenue, Mechanicsburg, Pennsylvania, or discuss their residence. Howard and Virginia Taylor, ""hen leaving the Meado,vs Unit, must be signed out and in. The log book is outside the entrance to the ~1eadows Unit (Ecumenical rule). 2. Hosts taking Howard and/ or Virginia"out must check with the nurse or medtech and take any medication along that is due to be administered (Ecumenical rule) . 3. For, Howard: Will need to take along nitrostat pills in case needed. Will need to follow the protocol established by Ecumenical for administration. Current protocol: When chest pain occurs, must check blood pressure PRIOR to administering nitrostat. The hosts will need to have a blood pressure machine with them in order to do this. (a) If systolic pressure is 90 or LESS, DO NOT administer nitrostat. Call 911. Transport to Harrisburg Hospital ER. (b) return. Be certain to return the medication directly to the medtech upon Note: Do not allow Howard to take possession of any medication. It must be held by one of the hosts personally, and also returned by one of them. 4. For Howard: Diet and/ or menu choices should be no or low sodium content because of medical condition. 5. Alcohol consumption should be limited to one alcoholic beverage with dinner (per conversation with Dr. Fugate, cardiologist, in June). 6. Guardian must be advised in advance who is transporting Ho\vard and/ or Virginia (w"ho is picking them up and dropping them off) and where they will be dining (this is only to assist in memory issues after the fact). - 7.
